The Chittenden Accountable Community for Health (CACH) is a cross-sector collaborative composed of individuals and organizations working together to improve the overall health and well-being of those who live and work in Chittenden County, VT. The goal of CACH is to address the top health priorities identified in the Community Health Needs Assessment (CHNA) which is conducted every 3 years. The most recent CHNA was released in 2022 which resulted in CACH forming three new teams to target the top three Health Priorities (Mental Health and Well-being, Housing and Cultural Humility and Inclusive Healthcare). In addition to the new 2022 CACH teams, the 2019 teams (based on the 2019 CHNA) are still hard at work and producing meaningful outcomes. Continue reading to explore the CACH Teams and their work.
